HANCOCK -- Those of you who live in Baraga, Gogebic, Houghton, Keweenaw and Ontonagon counties may receive a health survey in the mail.

The western Upper Peninsula Health Department randomly selected 8,000 households and needs your help collecting local health care data.  The survey will take no more than 15 minutes to fill out.  Questions range from health insurance coverage to eating habits, and all of the responses will remain anonymous.

"The quality of the data really hinders upon a good response rate," said Health Data Researcher Teresa Woods.  "It's really going to help health care providers.  We've got partnerships from the five regional hospitals and those providers, plus other planners, are going to be able to make use of that data to focus on health issues that are specific to our residents."

Every household that completes and sends back the survey by August 1 will be entered into a drawing to win one of eight $50 grocery store gift cards.
